I made a different approach and design the form and uploaded to the shopping cart root and inside the static page inserted the form code, without changes, with the HTML tools. And it worked just fine...
http://www.jimmythesailor.com/shop_coins/index.php this is the url for my new test shopping cart with a web form embedded. As you see I create a directory exclusively for the shop the "shop_coin" and inside I uploaded the form as it was built.

I used the same approach (embedding in the page with the HTML tool). Once I got the hang of how the program worked it was fairly easy to create three or four different forms and try them on the page (one at a time of course!). Thanks for your help!

Could a few of you try out the form just so I can be sure its working for people other than me? You should get a reply e-mail once you submit the form and be returned to the main page.

Tested and got the reply no problem, ended up in my spam, you may want to add a comment which I need to do also, that tells people to be sure to add the email address (and give the email address it's coming from) to their address book so it doesn't get marked as spam. Other than that looked good to me
