Is there anyway to add a file name of the *.png signature so it would either create a link in the csv file or at least get the file name into the csv so it can be referenced after the fact from an audit perspective "did joe actually sign the form"? yes here is the file (png)?

The signature part is completely separate from the form so it would take a substantial amout of coding to actually tie the two together in some way.

It would be a lot easier to do the online signature system instead. Add the following fields:

Signature Name: (Please type your name here to represent your signature)
[_______________________]

[ ] Filling in the signature box above with your name and checking this box binds you to the ...

Fill in whatever text you need it to represent for the check box. Make both fields required so they must do so to continue. You'll get that in email which is a printed and signed copy basically. Easier and just as legal as a copy of an image signature.
