Thanks for your response Eric.
Here is the link to the form http://www.highnetworth.store/testform.html
I also had a look at the php file that contains the api key, audience (list) key and field matching and all looks correct.
One thing I did notice in the php file is it contained information for an address field which is not in my form.

Have you tried manually deleting all files off the server and re-uploading?
I can't hear what I'm looking at.

I have multiple times. Every time I make a change to test, I delete the old files and folder before uploading again.
I paired the form down from multiple fields to only 3 to eliminate some possibilities but same result.
No data shows up in MailChimp.
I started a support ticket and uploaded my form config php file.

See what comes back.

The Support Topic is probably the proper thing if there's some bug. Can you share your project file?

I sent my project file to the support team and they used it to create the form on their (cc) server and it worked fine.
Still won't work on my server (in motion hosting). Talked to hosting tech support and they are at a loss as to why it won't populate mailchimp.
If I create a form in mailchimp and copy the html it gives me and paste into my site it works fine. My woo Commerce check out works fine as well. Both populate the mailchimp list fine.
No error log is ever created. Everything seems to work and even the confirmation emails go out from form builder but no date ever shows up. It's like it goes in the bit bucket never to be seen.
I told form builder to create a csv file and that works fine as well.
Site is running php 7.3
Pretty frustrating.

That's strange.
I'll look at my mailchimp example when I get a chance.

Sent all my files to CC support yesterday and am waiting to hear back.
In the mean time, I tried my form on a non secure site on the same server and still no luck.

I would think it has something to do with the server/hosting but the html generated by mailchimp and pasted into my site works fine as does my woo commerce.

I'll report back when I hear from CC support who by the way have been great to deal with.
