The form is corrupted on your server:

http://www.ovmp.org/form-contact-ovmp/f … ertest.php

At the top, you should see the program, version, build and operating system there but it is blank. I would suggest deleting the form, export and upload. It will not work until that information is displayed there at the top.

i am having the same issue. i've created two forms. one works and the other does not. it did work at one point. i don't want to re-export because i had to manually change a few things and that would be lost. the link to the broken form is: http://www.embracingourdifferences.org/ … index.html

the one that works is: http://www.embracingourdifferences.org/ … index.html

Either one or more files are missing or are corrupted. You can see that the Form Builder information is missing at the top:

Yes, that would be a similar issue.

Ok so if I compare the two scripts one from your website and one from mine here is what I noticed.

Your form:
<script type="text/javascript">document.write(unescape("%3Ciframe src=\"form/form.html\" width=\"600\" height=\"838\"allowtransparency=\"true\" scrolling=\"no\" frameborder=\"0\"%3E&lt;a href=\"form.php\" title=\"form\"&gt;Check out my CoffeeCup Form&lt;/a&gt;%3C/iframe%3E"));</script>
<noscript>
<iframe width="600" height="838" style="border:none; background:transparent; overflow:hidden;"
src="form/form.html">
&lt;a href="form.php" title="form"&gt;Check out my CoffeeCup Form&lt;/a&gt;
</iframe>
</noscript>

My form:
<script type="text/javascript">document.write(unescape("%3Ciframe id=\"fb_iframe\" src=\"LogoForm.php" + window.location.search + "\" width=\"100%\" height=\"1016\"allowtransparency=\"true\" scrolling=\"no\" frameborder=\"0\"%3E&lt;a href=\"LogoForm.php\" title=\"LogoForm\"&gt;Check out my CoffeeCup Form&lt;/a&gt;%3C/iframe%3E"));</script>
<noscript>
<iframe width="100%" height="1016" style="border:none; background:transparent; overflow:hidden;"
id="fb_iframe" src="LogoForm/LogoForm.html">
&lt;a href="LogoForm.php" title="LogoForm"&gt;Check out my CoffeeCup Form&lt;/a&gt;
</iframe>
</noscript>

Did you customize the output of this form after WFB ran its initial export? I notice a few references to html pages instead of the php pages that form builder exports. I am no expert coder but maybe the issue is here?

The files are corrupted on your server:

If you look at FormBuilder version:, it should output the version and os but that is blank. This shows it was a bad upload.
