The form is corrupted on your server:

At the top, you should see the program, version, build and operating system there but it is blank. I would suggest deleting the form, export and upload. It will not work until that information is displayed there at the top.

i am having the same issue. i've created two forms. one works and the other does not. it did work at one point. i don't want to re-export because i had to manually change a few things and that would be lost. the link to the broken form is: http://www.embracingourdifferences.org/ … index.html

Either one or more files are missing or are corrupted. You can see that the Form Builder information is missing at the top:

Ok so if I compare the two scripts one from your website and one from mine here is what I noticed.

Did you customize the output of this form after WFB ran its initial export? I notice a few references to html pages instead of the php pages that form builder exports. I am no expert coder but maybe the issue is here?

I just edited and uploaded a new form on my website and get the same error. I re-saved it and re-uploaded and get the same error. The form is located at: http://www.longislandrecyclers.com/cash-quote.html , can somone please provide a solution to this?

If you look at FormBuilder version:, it should output the version and os but that is blank. This shows it was a bad upload.
