You need to tell us more information about what the problem exactly is. Telling us is not working doesn't tell us what is wrong.

Form not going to correct page after submission was the problem....
When i fill out and submit the form it goes to this page:
http://www.beyonddiversity101.org/BD101Form.php
as opposed to:
http://www.beyonddiversity101.org/thanks.html.
Also i am not receiving the submitted form in my e-mail.
Is that enough info??

Actually somewhere along the line that's the page you told it to go to. If you look in your settings (or in the code itself that you put on the site) you'll see that it's going to BD101Form.php and in the code it looks like:
<form style="BACKGROUND-COLOR: transparent; WIDTH: 400px; WebkitTransform: "
id="docContainer" class="fb-toplabel fb-100-item-column fb-small selected-object"
enctype="multipart/form-data" method="post" action="../BD101Form.php" novalidate
data-form="manual_iframe">
Check your settings again and make sure where you've set it to send people too after sending. My guess is it's just a mistake in how you interpreted the information that should go in certain settings areas? Not sure, just know that that page is what it's being directed to.
