That should be NONE.
Searching on the net, it appears that SQL is not over keen on checkboxes. Majority of questions on the form that is not working are multiple choice checkboxes.

I'm sure this is a permission issue. I just don't see the checkboxes causing the problem when your not even getting that far.

I have five forms on the website and two are small and work but the non working one is longer.
I uploaded another form which is slightly longer than the non working one and that did not work either.
Seems to me that maybe the size of the forms could possibly be causing the issue.
