when i want submit the form appear the following error:
Your form could not be submitted for the following reason(s):
Failed to open database [1045] SQLSTATE[28000] [1045] Access denied for user ' bonoporm'@'server4.sitiodns.net' (using password: YES).

Sounds like you don't have your database info setup correct. Compare whats on the server to what you set in the form builder.

I put the following info:
Server: The ip of the server, its ok?
port: 3306, is the port that came default
username: user of the cpanel of my server
pass: pass of the cpanel of my server
database name: the data base name
table name: anyone

I would try asking on a wordpress forum. they should be able to tell you what your setting should be.
