I apologize if this has been addressed already, but on testing my survy on the new form builder, I found that a person can check yes and no for the same question. While it is possible to make sure that atleast one box is selected or even multiple boxes, how do I limit a person to check just ONE box?

Yes, that is how a checkbox works. You should instead be using radio button.
