You need a from email address in the confirmation.
I just tested the form and it submitted. I haven't received an email yet but will post if I do.

The server test looks good.

I use Bluehost and don't have any issues with my forms working.
Enter your email address for the confirmation and try one email address in the notification for testing.
If one email address doesn't work try a different one that uses a different email host. gmail vs yahoo vs AOL vs outlook.
I can't hear what I'm looking at.
It's easy to overlook something you're not looking for.

It seems to work ok with s-drive
Even with multiple emails in the notifications and cc

But I had to disable the form payments option, but that isn't or shouldn't be the reason.

I have sent you back the project file via WeTransfer look out for the email from them to download it

Hi Eric - From the Notifications tab, here is what I input in the field.
To: happyvince@gmail.com
CC: vincent@vqnphotos.com
BCC: blank
Reply-To: [email]
From: happyvince@gmail.com (doesn't receive any email from Gmail). Then I input my domain email, which is vincent@vqnphotos.com (did not receive anything).

Wayan - I downloaded the form you tested and placed on my computer. I have enabled the Payment Options and it does not send to my email(s).

Conclusion: The form needs to have a payments and the notifications tab needs to work. Based on my input above, I don't know what I need to do. Please help me out! I need this form to be up ASAP.

I tested the form and it works. I try to submitted 10 times with different emails and it works flawlessly.

On the Payment tab.
When I enable the checkbox for Enable form payments and submit the form, it shows the next step for payment only. I did not received any notifications email. Is it a cliche in the CoffeeCup Web Form Builder version 2.9 Build 5509?

I minute I enable the checkbox payment, I did not received any notification email. Is there any other ways to work around? Please advise!

Well if it works with the payments disabled and doesn't work with it enabled then that has to be the problem, I cannot check that out as it is your payment gateway the only thing I can suggest is you double check the settings for your payment gateway integration as per your payments provider.
I am sorry I can't help with this I don't use forms for payments so wouldn't even know where to start looking or what to look for.
