It didn't like to be saved as html file because of the php script.
I did it anyway and the form didn't display at all.

I just tested by deleting the 004WZ.html file off my server and it would hang again. Uploaded it back up and the form works as expected.

I'll take a look at the coffeecup scripts to see where it's referencing the 004WZ.html maybe it can be changed.

Found it.
Line 62 of the formpage.cls.php page
It's in the directory 004WZ/fbapp/php/formpage.cls.php
Change this
to

Make sure to put that reset script in there if the form is holding values when you go back to it.
