When I submit from the form, it is not adding a column and saving the file name being uploaded to the MySQL database. It does add other columns and saves their values in the MySQL table. Is it not supposed to save the file it I have the STORE IN DATABASE enabled?
I noticed that two other tables were created by WFB called DBNAME_upload and DBNAME_uploadrefs but neither of these have anything written to them. I have attached a screen shot of the WFB form field properties.

Are you by any chance also saving it as a csv? I thought there was an issue a long time ago with that combo but I thought it was fixed.

No I am not saving it as a CSV. Only adding information to the MySQL DB. I have opened a support ticket in my room.
