Can you upload one of the forms that aren't working to your own server so we can see it there?

Quick question as I have made the mistake too. When you start selecting your display rule and you move down all the choices are you hitting the last button at the bottom to add the code to the form? I am not on a machine that has WFB installed so I cannot screenshot it from here.
