Hi Patrick,
I would ask you if all the files are in the same directory, as I cannot check at the moment, and as far as the alignment goes have you tried using the arrow keys to nudge them into place.

I did try to submitt your form however and did get an error as you stated.

Patrick,

Open your form builder.

Click on Edit.
Click on Grid Settings.
Remove the check-mark from Snap to Grid.

This about text forms

I use these forms in order for my text not to be coming from the far left margin and I don't like the center position either.

Sometimes when I use them they work, sometimes not and sometimes when they work, later they wont.

That is not a form but a table. BTW, have you seen that the thread you replied to is over three years old? In the meantime we have a new form builder which is different from the one discussed here.
