Setting up database for Web Form...

I have created a form for a club I am the membership secretary for and am creating the database for the elements on the form.
I am using tick boxes for the answer to some of the questions and am not sure how-to setup the Dropdown fields in the database. Do you save them as just text or are they Boolean values?
I'm guessing text as you can have more than 2 in the dropdown field.
I am also using checkboxes, so what do you setup in the database for these fields?

Hi Rocky,
Don't setup the table. Enter your database user info and the name of the database. Then enter the name you would like to user for your database table for the form. The first time you submit the form the table will be created for you.
I can't hear what I'm looking at.
It's easy to overlook something you're not looking for.
